I accept multiple forms of payment including:
HSA/FSA, check, credit cards and payment plans.
***I will not talk to your insurance company or HR department nor will I fill out ANY tax forms because I do not work for either of them. However, I will give you a receipt after you complete your payments for you to use to get reimbursed.
***I DO NOT have any billing codes because I am not a medical provider.
***I do not contract with the On Up Doula Program or any insurance company.
However, I will have a conversation with you about discussing reimbursement with your insurance/employer
to get reimbursed for your payment to me.
Don't hesitate to ask questions on how to get reimbursed from your insurance\\employer.
